The Program Coordinator provides hands-on, person-centered support to the people we serve and leads the Direct Support Professionals in program implementation. The Program Coordinator provides support to the Program Manager by completing administrative related duties and fulfilling direct care shifts.

Key Responsibilities:

  • Provides direct care to individuals served under limited supervision
  • Mentors Direct Support Professionals
  • Follows programming outlined in the Individual Abuse Prevention Plan (IAPP), Self-Monitoring Assessment (SMA), and Coordinated Services and Support Plan and Addendum (CSSP)
  • Monitors and maintains safety of the individuals served
  • Drives and accompanies individuals served to appointments
  • Maintains home and needs of individuals served by cooking, cleaning, etc.
  • Documents detailed narratives and behavioral data daily
  • Administers medication, completes Medication Administration Record (MAR), orders medication, and resolves any pharmacy issues
  • Models behavior and mentor’s individuals served to increase daily living skills
  • De-escalates maladaptive behavior
  • Preforms RCC trained manual restraints (EUMR) during behavioral incidents
  • Participates in activities and social experiences within the community with individuals
  • Collaborates with individuals served teams to write new goals or modify existing ones
  • Shares behavioral, health, and program concerns with Program Manager
  • Attends and completes all required trainings, including staff meetings
  • Assists Program Manager with staff training
  • Provides indirect supervision by leading the Direct Support Professionals and giving DSP’s performance feedback when appropriate
  • Audits documentation to ensure the Direct Support Professionals are completing tasks
  • Develops meal plans and completes grocery shopping
  • Assists Program Manager with budgeting and ensures program stays within budget
  • Assists Program Manager with scheduling and other schedule related items
  • Fulfills a rotating on-call schedule for weekends and holiday’s

Requirements

Experience

  • 2+ years of experience working with individuals who have disabilities required
  • 1 year of post-secondary education and/or working towards a degree in Human Services preferred
  • Basic math skills
  • Basic domestic skills (cooking, cleaning, laundry)
  • Judgment and decision-making skills to handle crisis situations and day-to-day events

Physical Requirements

  • Walking on an on-going basis
  • Push a wheelchair up to 250lbs on occasion (site specific)
  • Support individual by holding them upright up to 250lbs on occasion (site specific)
  • Climb stairs occasionally (site specific)
  • Reach and grip with hands (frequently)
  • Stoop, kneel, crouch on occasion
  • Peripheral vision on an on-going basis

Environmental Factors

  • Works with others on an on-going basis
  • Has public contact on occasion
  • Shift work usually
  • Behavioral incidents that may require physical intervention on occasion

Non-Negotiable Hiring Criteria

  • Ability to write narratives in grammatically correct sentences in English
  • Communication skills adequate to relay information in English
  • Valid Driver’s License with a satisfactory record
  • Maintains vehicle insurance
  • Excellent time management skills
  • Proficient with using computers
  • Positive attitude
  • Client focused
  • Role model for other staff
